We offer care management assistance 24 hours, 7 days a week. Through our beeper program, a care manager can respond to your needs providing advocacy and security until the family can arrive to assist
Our services include:
24 hour access to a care manager
Initial Consultation to organize essential information which allows the care manager to be an effective representative; insurance, medications, medial history, physicians, family contacts, power of attorney, living will status, and a home safety issues
Quarterly meetings scheduled with your care manager
This program is billed annually.
If called for, emergency services are billed on an hourly rate.
Placement Assistant A care manager knows the available facilities in their area well and can assure placement is appropriate for your loved one.
Crisis Intervention We offer care management assistance 24 hours, 7 days a week. A care manager can respond to your needs providing advocacy and security until the family can arrive to assist.
Adjustment Support Adjusting to new living arrangements is difficult and can produce fear and anxiety about the future. We assist by preparing the individual for these changes. This counseling explores and promotes the understanding of the emotions that accompany transitions.
